Unfortunately, such realism is lacking in the base game, which is why you need Zero’s Mental Health mod. It’s a thoughtfully built system that brings psychological depth into The Sims 4. With this mod, Sims can develop various mental conditions like anxiety, depression, and PTSD, each with unique symptoms, emotional buffs, and behavioral changes. Your Sim can get diagnosed with 11 possible disorders, which actively affect their moods, autonomy, and social interactions. Once diagnosed, your Sim can choose therapy and medication options to help manage their mental health. This allows for dynamic recovery and setbacks depending on how you play. This is one of my favorite Sims 4 health mods as it supports storytelling-based gameplay by triggering mental health events after trauma. For instance, if your Sim loses their job, they can become overly anxious about life and their future. I highly recommend getting the mod if you want to represent mental wellness in a nuanced, sensitive, and gameplay-integrated way.

How to Install

Follow these steps to install Mental Health Mod on your Sims 4 game:

    Download the mod file from the link above. Sims 4 mods typically come as .package or .ts4script files.

    Open your Sims 4 Mods folder. The default location is: Documents/Electronic Arts/The Sims 4/Mods

    Place the downloaded file directly into the Mods folder. You can organize mods into subfolders up to one level deep.

    Launch The Sims 4 and go to Game Options > Other. Make sure "Enable Custom Content and Mods" is checked.
