Wicked Whims is best known for its adult content, but it also includes a surprisingly detailed and realistic menstrual cycle system. With this feature enabled, Sims who menstruate will go through real-life monthly cycles complete with mood swings, cramps, and bleeding. You’ll need to stock up on pads or tampons, and your Sims can experience discomfort, irritability, or low energy during their period. The cycle also affects fertility, so you’ll have to pay attention if you’re trying for a baby (or trying not to). This is one of my go-to Sims 4 health mods because it’s fully customizable in the settings. You can adjust length, symptoms, and even fertility tracking based on your gameplay style. The mod also introduces a Period Tracker app you can use to stay on top of your monthly cycle. This comes in handy when you’re trying to conceive.
Follow these steps to install Wicked Whims – Menstrual Cycle Feature on your Sims 4 game:
Download the mod file from the link above. Sims 4 mods typically come as .package or .ts4script files.
Open your Sims 4 Mods folder. The default location is: Documents/Electronic Arts/The Sims 4/Mods
Place the downloaded file directly into the Mods folder. You can organize mods into subfolders up to one level deep.
Launch The Sims 4 and go to Game Options > Other. Make sure "Enable Custom Content and Mods" is checked.
Restart the game if it was already running. The mod should now appear in your game.
Important: Always back up your save files before installing new mods. After Sims 4 updates, mods may need to be re-downloaded or updated by the creator.

Pro tip: If a mod stops working after a game update, check the creator's page for an updated version. Broken mods can cause the game to behave unexpectedly.
